Texas Medical Concierge: Personalized Medicine Plan (Integrated Healthcare Services)

Personalized medicine (Integrated healthcare services)

Personalized medicine, also referred to as precision medicine, is an emerging field in medicine that uses advanced diagnostic tools to identify genetic markers (specific biological markers) to determine which medical treatment and therapy will best suit for every individual patient. By using this genetic information of an individual through his or her medical records, personalized medicine allows doctors to develop a specific prevention and treatment plan for every individual.

As discussed in our previous blogs, Texas Medical Concierge located at ITC Maurya, New Delhi has introduced personalized medicine plan which covers in a 3-step approach.

The plan includes following three steps:

  1. Medical history & lifestyle evaluation
  2. Pathology & radiology investigations
  3. Integrated health consultancy report

The first two approaches are already discussed in detail in our previous blogs. This blog will mainly highlight on how integrated healthcare services can help in the treatment of patients.

Integrated healthcare services

Integrated healthcare also known as interdisciplinary healthcare, is an approach which is characterized by a high degree of collaboration and communication among various healthcare professionals. This approach is unique as this requires sharing of information among the healthcare professionals related to patient care which helps in development of a comprehensive treatment plan. Integrated healthcare service addresses biological, psychological and also occupational-related problems of the patient that helps in the treatment of particular disease.

How integrated services works?

Integrated healthcare services include multiple treatment procedures to benefit patients to overcome their health-related problems across their lifespan. These include: primary care, specialized medical care (such as rehabilitation units, surgical centers, cardiology center, etc.), community-based healthcare services and social services.

Integrated healthcare services starts with a good primary healthcare followed by comprehensive healthcare services that are well coordinated with proper communications among healthcare providers. The most essential part of integrated healthcare delivery is a high-performing primary care who can manage to give seamless, well-coordinated delivery and serve high quality care. Therefore, the main goal of having integrated services is to ensure optimal care and avoid errors that can results from the overuse, underuse or misuse of diagnostic tests, procedures or prescribed medications.

Texas Medical Concierge: Personalized Medicine Plan (Pathology & Radiology Investigations)

Personalized Medicine Plan (Pathology & Radiology investigations)

Personalized medicine is an upcoming and a promising concept, which involves the application of genetic information to predict disease development, influence decisions about lifestyle choices and tailor preventative interventions or medical treatment to the individual needs of each patient. Thus, personalized medicine will allow screening, early intervention and treatment to be concentrated on those patients who will benefit more and thereby reducing the side-effects of unnecessary medicines.

Personalized medicine approach is an innovative, more efficient and patient-centered alternative when compared to traditional one-size-fits-all approach. Therefore, some of the major medical organizations like the American Medical Association and the American College of Physicians have now shifted  to periodic screenings tailored to individual patient’s needs instead of a routine physicals every year.

Texas Medical Concierge located at ITC Maurya, New Delhi has brought the personalized medicine plan to India (please refer our previous blog). As mentioned previously, this plan is covered in a following 3-step approach:

  1. Medical history & lifestyle evaluation
  2. Pathology & radiology investigations
  3. Integrated health consultancy report

In this blog we will concentrate mainly on the second approach i.e. Pathology & radiology investigation.

Pathology & radiology investigations

Pathology testing plays an essential role in screening of diseases and in designing a proper disease prevention programs for every individual patient. Pathology screening will also help physicians to find out how the current treatment procedure is working on the patient.

Value of pathology

Proper pathology testing services helps in early detection and prevention of diseases and thus ensures timely clinical diagnosis and treatment at all stages of a particular disease. Some of the examples where pathology examination plays crucial role are:

  • As a part of maternal-antenatal care, pathology testing helps to detect and prevent possible health problems of both mother and baby e.g. mother may be tested for iron levels (anemia) and glucose levels (gestational diabetes), whereas the fetus may be tested for any chromosomal abnormalities.
  • Cancers – where pathology testing is an absolute requirement for diagnosis of new cases and is used to determine the best treatment options, predict prognosis, monitor disease progress and evaluate effectiveness of the treatment.
  • Cardiovascular diseases – where pathology testing is used to detect early symptoms in those having higher risks and drug sensitivity e.g. cholesterol levels, lipid profile, etc.
  • Other medical conditions –pathology testing plays a crucial role in detecting various other medical conditions such as in diabetes, heart problems, thyroid conditions, liver disease and arthritis.

Like pathology, radiology testing is also an essential part of screening procedure. Radiology investigation uses diagnostic imaging tests and interventional procedures that involve the use of X-rays, ultrasounds and magnetic resonance imaging equipment’s, which helps in detection of diseases.

Different types of radiology investigations

Radiography procedure can be carried out by using an analog film-screen system or in digital format by using computed radiography (CR) with a latent image or by using direct radiography (DR) without a latent image.

Common radiology investigations include MRI scans, CT scan and using plain X-rays.

Magnetic resonance imaging (MRI) – is an imaging technique which uses magnetic fields, radiofrequencies and a computer to take highly detailed images of the human body.

Computed tomography scans (CT scans) – uses a series of X-ray images of different parts of human body which helps the radiologist to have a detailed picture of the structures.

X-rays – this investigation involves a brief exposure of a specific part of a body to an X-ray radiation. Although, X-rays do not show soft tissues clearly, however, they are very useful in the assessment of bone structures.

Pathology and radiology investigations thus help doctors and physicians to make an accurate decision about the diagnosis and treatment of diseases.

Personalized medicine plan introduced at Texas Medical Concierge includes Pathology & Radiology Investigations as a second aspect which include taking up of the following tests:

  1. Follow-up tests – The follow-up tests are done for any abnormal values detected in the previous tests.
  2. Preventive tests – These tests are done mainly based on the risk factors like age, family history, living conditions, etc.
  3. Chief complaints and ailments – This session involves studying the medical symptoms or signs that are highlighted during the history session of a patient and thus indicating for further evaluation.

Texas Medical Concierge: Personalized Medicine Plan (Medical History & Lifestyle Evaluation)

Personalized medicine, also referred to as precision medicine is an emerging field of medicine that uses advanced diagnostic tools to identify genetic markers (specific biological markers) to determine which medical treatment and therapy will best suit an individual patient. By using genetic information of an individual available through his or her medical records, personalized medicine allows doctors to develop a specific and targeted prevention and treatment plan for an individual.

Texas Medical Concierge at ITC Maurya, New Delhi has brought Personalized Medicine plan to India. Being one to keep with the times, we have come up with Personalized Medicine Plan which is covered in a 3 step approach:

This plan includes three steps as shown in the figure below:

  1. Medical History & lifestyle Evaluation
  2. Pathology & Radiology Investigations
  3. Integrated Health Consultancy Report

The 3 –step approach of Personalized Medicine Plan at Texas Medical Concierge

So what is medical history and lifestyle evaluation and how is it done?

A commonly stated aphorism among the doctors is ‘Listen to your patients, they are telling you the diagnosis”. Taking medical history is actually sharing of experiences and views between a patient and the doctor. A good history taking session is one where a patient reveals his understanding, concerns, and expectations to the doctor who in turn listens to the patient carefully. It is a crucial step before the doctor arrives at a proper diagnosis and a significant milestone in developing and maintaining a good medical history record.

Medical history or case history of a patient is the information obtained by a physician by asking specific questions with the aim of obtaining specific information useful in formulating a diagnosis and treatment care for the patient. Taking proper medical information from a patient is an art and requires good amount of skill, practice, patience, and depth of knowledge in the subject. It is important for the physician to possess good communications skills to develop a rapport with the patients. Lifestyle is the way a person lives and his or her association with smoking, drugs, and alcohol. Lifestyle evaluation enables the physician to know their level of physical activity and chances of their susceptibility to lifestyle diseases like atherosclerosis.

Why is it medical history & lifestyle evaluation important?

A medical history will help to identify if a patient is already suffering from heart disease, high blood pressure, stroke, certain cancers or diabetes to name a few. These disorders are influenced by combination of genetic factors, environmental conditions and one’s lifestyle choices commonly known as genotype and phenotype among the medical fraternity.

Medical history will also provide an insight into patient’s risk factors for particular diseases as well as information about his or her past diseases, surgeries, allergies, drug sensitivity etc. For example, for patients with a higher risk of certain cancers a physician may keenly monitor and recommend periodic screening (such as mammography or colonoscopy) for detection of cancer at an early stage.

Lifestyles diseases are those diseases which results from individual’s lifestyle choices such as from eating habits, lack of exercise etc. Example of some common lifestyle diseases are heart disease, atherosclerosis, cancer, obesity, type 2 diabetes etc. Understanding about one’s lifestyle and incorporating better lifestyle changes such as healthier diet, regular exercise and quitting smoking and excessive alcohol consumption will help people to lower chances of developing such diseases.

Personalized Medicine: Personalized fit medical treatment

1st Blog image

If you have read the lines “the right drug for the right patient with right dose at the right time”, you have read it right! In medical terms, this is defined as “Personalized medicine” or “Precision medicine”.  Scientifically personalized medicine is defined as an emerging practice of medicine that uses an individual’s genetic profile to guide decisions made in regard to the prevention, diagnosis, and treatment of a disease. According to the Forbes, the modern medical treatments are shifting from “one size fits all” approach to a more personalized fit!

So what is the basis for personalized medicine? If each individual is one of a kind, then the risk of disease susceptibility and response to treatment will also differ. Would not it be just great to have a treatment specially designed for your personality?

 Development of Personalized Medicine Concept

Is the concept of personal medicine new? Not at least for India! Here, the concept of personalized medicine based on genetic variations is not new and is existing from years through the practice of traditional medicine or “Ayurveda”. However, we are talking about modern medicine here.

The giant leap for personalized medicine being precise was based on the use of genetic information from patients to predict susceptibility, prevention, prognosis or appropriate therapy for individual patient.

The success of personalized medicine will depend on better understanding on the study of genetic mapping. Genetic profiling of a patient which includes prediction of disease risk and drug response will help in understanding pathogenesis of disease better and thus the success of personalized medicine depend on the study of genetic mapping.

Researchers are making considerable progress in many areas of personalized medicine such as for treatment of various cancers, mental illness, HIV-AIDS etc. These research works are very crucial as there are significant patient-to-patient variability in response to various drugs, for example the response to psychoactive drugs varies in case of mental illness.

A recent breakthrough in the use of biomarkers for personalized medicine was the FDA’s approval of a drug called kalydeco (ivacaftor) that is used in treatment of rare form of cystic fibrosis in patients (those suffering from specific G551D mutation in cystic Fibrosis transmembrane Regulator (CFTR) gene).

 Importance of Personalized medicine

Personalized drug therapy is one of the most sophisticated forms of medicine which uses “biological indicators” or “biomarkers” such as variants of DNA sequences, the levels of certain enzymes or the presence or absence of drug receptors as indicators. This helps researchers to understand how patients should be treated for different medical conditions and to estimate whether the intervention would be effective or not.

It  will enable scientists and doctors to develop an exact drug therapy for patients which will be matched to the best therapy based on their genetic makeup and other predictive factors.
